Chapter 1 - The Woman Behind the Mafia Boss's Fear

A woman fell beside a wheelchair in a hospital garage, screaming: "Run, Rosa!" When Mafia boss Dante Marino arrived, he found Clara's medical badge covered in blood beside the abandoned wheelchair. On the back of the badge, three words were written: "Protect Her Always." Dante realized Clara was never just a therapist.
The first thing I remembered was the sound of gunfire.
Not the pain.
Not the fear.
The sound.
Five shots echoing through the underground hospital garage.
Five explosions that changed my life forever.
But before I became the woman lying in Dante Marino's arms, before the entire Marino family learned my name, I was just Clara Hughes.
A physical therapist from Queens.
A woman who counted every dollar before buying groceries.
A woman who learned how to smile while drowning in debt.
My father had been gone for eleven months.
Leukemia took him slowly.
The hospital bills came faster.
Every month, another envelope arrived.
Every month, another reminder that losing someone did not mean losing responsibility.
I had almost accepted that my life would always be about surviving.
Then Dante Marino's employee walked into my clinic.
He didn't introduce himself.
He didn't ask permission.
He placed a folder on my desk and looked at me like he already knew everything.
"You are Clara Hughes?"
I looked at the stranger in the expensive suit.
"Yes."
"Good."
I opened the folder.
My education records.
My work history.
My father's illness.
My financial problems.
Everything.
I closed it immediately.
"Is this supposed to make me feel comfortable?"
"No."
His honesty surprised me.
"What is this?"
"A job offer."
I almost laughed.
Until I saw the number.
One hundred thousand dollars.
Three months.
Live-in medical care.
I looked back at him.
"Who needs therapy?"
"Rosa Marino."
The name froze my smile.
Everyone in New York knew Dante Marino.
Some knew him as a businessman.
Others knew him as a man nobody wanted to disappoint.
I knew him as a warning.
"I can't."
The man looked at my worn-out shoes.
Then my broken phone.
Then the unpaid bill sticking out of my bag.
"You can."
I hated that he was right.
Two days later, I drove through the gates of the Marino estate.
The first thing I noticed was the security.
The second thing I noticed was that nobody there looked relaxed.
Men in black suits stood around the property.
Cars followed every movement.
Every door had someone watching.
I should have left.
But then I met Rosa.
She was sitting near a large window, looking outside.
An elegant woman in her seventies.
Silver hair.
Sharp eyes.
A body weakened by a stroke, but a mind that was still stronger than everyone around her.
"You are Clara?"
"Yes."
She looked me up and down.
"You look too young to fix broken people."
I smiled.
"And you look too stubborn to stay in that wheelchair."
For three seconds, the room was silent.
Then Rosa laughed.
A real laugh.
That was the first time I saw someone inside the Marino house make a normal sound.
From that day, everything changed.
Rosa wasn't just my patient.
She became family.
We practiced walking.
We argued.
We laughed.
She told me stories about Dante.
Mostly embarrassing ones.
"He was a serious child."
"I can believe that."
"No, you can't."
I smiled.
Because I had already noticed something.
Everyone feared Dante Marino.
Except his mother.
And eventually...
me.
Dante appeared at the estate every night.
At first, he only checked on Rosa.
Then he started asking about her progress.
Then he started asking about me.
"You worked twelve hours today?"
"I'm fine."
"That's not what I asked."
I looked at him.
"You always talk like that?"
"Like what?"
"Like every sentence is an order."
He stared at me.
Nobody spoke to him that way.
But instead of getting angry...
he smiled.
Barely.
A tiny expression most people probably never saw.
That was the dangerous part.
Because behind the rumors, behind the power, behind the fear...
there was a son who was terrified of losing his mother.
And somehow, I started seeing that man.
Not the reputation.
Not the name.
Him.
Weeks passed.
Rosa improved.
Dante changed.
His men noticed.
His enemies noticed.
And I noticed something else.
Someone inside the Marino family was watching me.
At first, I ignored it.
Then strange things started happening.
My medication records were moved.
My schedule was changed without my knowledge.
Someone entered Rosa's room while I was away.
Dante didn't believe it was a coincidence.
"You need more security."
"I am not a prisoner."
"You don't understand who you are around."
I looked at him.
"No. You don't understand who I am."
That surprised him.
"What does that mean?"
"It means I know exactly who I am protecting."
For the first time, Dante had no answer.
Then came the hospital appointment.
Rosa needed another examination.
Dante wanted to cancel.
"The risk is too high."
"The risk of staying home is also real."
His jaw tightened.
"You think I don't know that?"
"No."
I stepped closer.
"I think you are scared."
Nobody talked to Dante Marino like that.
Nobody.
But he didn't walk away.
The next morning, three black SUVs arrived.
Rosa sat between us.
Dante watched every person in the garage.
Something felt wrong.
Too quiet.
Too empty.
Then the lights flickered.
Once.
Twice.
Dante immediately stood.
"Everyone down."
A second later...
A red laser appeared on Rosa's chest.
And Clara was the only person who saw it.
I didn't think.
I didn't calculate.
I didn't care about the consequences.
I only saw Rosa.
The woman who treated me like family.
The woman who gave me a reason to believe my life was more than debt and survival.
So I moved.
I pushed her wheelchair away.
I stepped forward.
And then the world exploded.
The first bullet hit my shoulder.
The second hit my side.
The third sent me to the ground.
But before I lost control of my body...
I heard Dante scream my name.
Not as a boss.
Not as a powerful man.
As someone who was terrified.
And when I opened my eyes for one final second, I saw Dante Marino running toward me.
May you like
But behind him...
someone inside the Marino family was already walking away.
