Chapter 1 - The Woman He Refused to Release

The most feared mafia boss in New York had survived twelve bullets, a burning car, and a betrayal from someone inside his own family. But when Luca Moretti finally opened his eyes, he did not ask who had tried to kill him. He tightened his fingers around the hand of an ordinary nurse and gave an order that made every armed man in the room obey without hesitation.
“Everyone out.”
The heart monitor beside his bed continued its steady rhythm, but the luxurious hospital suite became so silent that Elena Carter could hear her own frightened breathing.
Luca lay against the white pillows with his tattooed chest wrapped in bandages. Bruises darkened one side of his face, and a thin scar cut across his cheek beneath the stubble along his jaw. He had been unconscious for almost forty-eight hours, yet there was nothing weak about the way his hazel eyes moved across the room.
Four men in black suits stood near the door.
A blonde woman in an ivory designer dress stood at the foot of his bed.
And Elena stood beside him, her hand trapped inside his powerful grip.
The blonde woman stared at their joined hands as though she had discovered a knife buried in her chest.
“Luca,” she whispered. “What are you doing?”
His gaze never left Elena.
“Don’t leave.”
Elena swallowed. “Mr. Moretti, you need to remain calm. Your wounds—”
“I know my wounds.”
His voice was low and rough, but every man in the room straightened as soon as he spoke.
The blonde woman stepped forward. Diamonds sparkled at her ears and around her throat. She looked elegant enough to belong at a royal wedding, but anger had already begun to crack the polished mask on her face.
“I’m Celeste Romano,” she said to Elena. “His fiancée.”
Elena gently tried to pull her hand away.
Luca’s grip tightened.
Not enough to hurt her.
Only enough to make his intention clear.
Celeste’s lips parted in disbelief. “Luca, your doctors said you might be confused.”
“I’m not confused.”
“You have been unconscious for two days.”
“And she was here for both of them.”
Elena felt everyone’s attention settle on her.
She had not known who Luca truly was when the ambulance brought him into Saint Catherine’s private emergency wing after midnight. His clothes had been soaked in blood. His pulse had disappeared twice before they reached surgery. Armed men had filled the hallway while surgeons fought to remove bullets from his chest, side, and shoulder.
Elena had assumed he was a wealthy businessman.
Then she heard the whispers.
Luca Moretti.
The man newspapers called a real-estate investor.
The man federal investigators had failed to convict for more than a decade.
The man rumored to control the docks, construction unions, nightclubs, private security companies, and half the politicians who publicly condemned organized crime.
Even unconscious, Luca had inspired fear.
Doctors lowered their voices around his bed.
Administrators appeared every hour to ask whether he was receiving the best care.
Police officers waited downstairs but never entered his suite.
Elena had treated him as she treated every patient.
She had cleaned the blood from his skin. Adjusted his medication. Monitored his breathing. Spoken to him when his fever rose, although she had not believed he could hear her.
At four that morning, when his heart rhythm became unstable, she had stayed beside him and whispered, “You don’t get to die after making all of us work this hard.”
His fingers had moved.
Barely.
Then they had closed around hers.
He had not released her since.
Celeste looked at the bodyguards. “Why are you standing there? Remove this woman.”
The men did not move.
Luca finally turned his head.
His expression changed so subtly that Elena might have missed it had she not been watching him closely. The warmth disappeared from his eyes. His face became still, and a terrible calm entered his voice.
“Matteo.”
The tallest bodyguard lowered his head. “Boss.”
“Take Ms. Romano home.”
Celeste stared at him. “You can’t be serious.”
“Very.”
“I came here because your mother called me. Our engagement announcement is in three weeks.”
“There will be no announcement.”
The words landed without warning.
Celeste’s face lost its color.
Even the bodyguards looked surprised.
She stepped closer to the bed. “You were shot. You’re medicated. You don’t know what you’re saying.”
Luca lifted his hand slightly, revealing that he was still holding Elena’s.
“I know exactly what I’m saying.”
“You’re ending our engagement because a nurse held your hand?”
“No.” His eyes sharpened. “I’m ending it because the first thing I remembered after waking was that I never wanted to marry you.”
Celeste’s humiliation filled the room.
Elena wished the marble floor would open beneath her.
This had nothing to do with her, yet Celeste looked at her with hatred so intense that it felt personal.
“You’ll regret this,” Celeste said.
Luca’s mouth curved into the faintest smile.
It was not a kind smile.
“People who threaten me usually don’t get the opportunity to watch me regret anything.”
Matteo opened the door. “Ms. Romano.”
Celeste’s hands trembled at her sides, but she raised her chin and walked out. The other men followed, leaving Matteo near the entrance.
Luca looked at him.
“Everyone.”
Matteo hesitated. “Boss, security protocol requires—”
“Outside the door.”
“Yes, Boss.”
When the door closed, Elena was alone with the man half of New York feared.
She finally removed her hand from his.
Luca allowed it this time.
“You embarrassed her,” Elena said quietly.
“She’ll survive.”
“You ended an engagement five minutes after waking from surgery.”
“I should have ended it months ago.”
“That still doesn’t explain why you’re holding a stranger hostage beside your bed.”
His eyes moved over her face.
“You’re not a stranger.”
“You don’t know me.”
“You stayed.”
“I’m your nurse.”
“Other nurses changed shifts.”
“I was assigned to your overnight care.”
“You argued with a surgeon when he wanted to delay the second procedure.”
Elena froze.
“You were unconscious.”
“I heard you.”
Her expression softened despite herself. “You shouldn’t have been able to.”
“I heard your voice every time I started slipping away.”
For the first time since he woke, Luca looked vulnerable.
It lasted less than a second.
Then the powerful man returned.
“You told me I didn’t get to die.”
“I say strange things when I’m tired.”
“You threatened me.”
“I encouraged you.”
“You threatened a Moretti.”
“I didn’t know who you were.”
“Would it have changed anything?”
Elena considered the question.
“No.”
Something warm entered his gaze.
“That’s why you’re still here.”
She checked the monitor to avoid looking at him. “You need rest.”
“I’ve rested enough.”
“You were clinically dead for nearly two minutes.”
“So I’ve been told.”
“Most people would be frightened by that.”
“Most people don’t have my enemies.”
His blunt answer reminded her exactly who he was.
Elena picked up the chart from the side table. “The police will want to speak with you.”
“They can want.”
“You were attacked in the middle of Manhattan.”
“It wasn’t an attack.”
She looked at him.
“What was it?”
“A message.”
“From whom?”
“I’ll know soon.”
There was no doubt in his tone. He did not sound like a victim hoping justice would find him. He sounded like a king waiting for the name of a rebel before ordering an execution.
Elena replaced the chart.
“I’m going to get your doctor.”
Luca reached for her wrist.
The movement made pain flash across his face.
She immediately leaned closer. “Don’t move like that.”
“Promise you’ll come back.”
The request surprised her more than his command had.
“You have an entire hospital taking care of you.”
“I asked you.”
Elena looked at his fingers around her wrist. They were scarred across the knuckles. The hand belonged to a man who had probably hurt people. Perhaps many people.
Yet his touch remained careful.
“I’ll come back,” she said.
He released her.
Outside the suite, Matteo stood with two guards. Celeste was gone, but a woman in her early sixties had arrived. She wore a pale gray suit, pearl earrings, and an expression that made the entire hallway seem colder.
“Elena Carter?” she asked.
Elena stopped.
“Yes.”
“I am Isabella Moretti. Luca’s mother.”
There was no warmth in the introduction.
Elena straightened. “Your son is awake. His vital signs are stable, but the doctor needs to examine—”
“I didn’t ask about his vital signs.”
Isabella glanced at the closed door.
“I want to know why my son dismissed his fiancée while holding your hand.”
Elena felt the guards listening.
“You’ll need to ask him.”
“I am asking you.”
“I don’t know why he made that decision.”
“My son does not make decisions without a reason.”
“He woke after severe trauma. His behavior may be influenced by medication.”
“You don’t believe that.”
Elena remained silent.
Isabella stepped closer. Her perfume was expensive and subtle.
“You appear intelligent, Miss Carter, so I’ll speak plainly. My family attracts people who mistake proximity for opportunity. Nurses, assistants, employees, women who imagine one moment of attention can transform their lives.”
“I don’t want anything from your son.”
“They all say that.”
“I didn’t know his name until after surgery.”
“But you know it now.”
Elena met her eyes. “Yes. And I’m beginning to understand why everyone around him speaks as though they’re walking through a minefield.”
One guard quickly looked away, hiding what might have been amusement.
Isabella’s expression hardened.
“Be careful.”
“Is that medical advice?”
“It is the only warning you will receive.”
The door opened behind them.
Luca stood there.
Barefoot.
Shirtless.
One hand gripping the IV pole.
Blood had already begun to spread through the bandage near his ribs.
Elena’s heart nearly stopped.
“What are you doing?” she demanded, rushing toward him.
His gaze rested on his mother.
“Threatening my nurse?”
Isabella’s face changed. “Luca, get back into bed.”
“Answer me.”
“I was clarifying her position.”
“Her position is beside me.”
Elena placed an arm around his waist, trying to support him without pressing against his wound. “You’re bleeding. You need to lie down now.”
Luca allowed her to guide him back into the room.
His mother followed.
“Leave,” he said.
“Luca—”
“I nearly died, Mother. Don’t make my first day back unpleasant.”
Isabella looked from him to Elena.
“You have no idea what you’re doing.”
“I usually do.”
“What about the Romano alliance?”
“Let them find another way to feel important.”
“They will take this as an insult.”
“It was intended as one.”
Elena helped him lower himself onto the mattress. His breathing became shallow, but he did not show pain in front of his mother.
Isabella approached the bed. “Your father spent thirty years building peace between our families.”
“My father is dead.”
“And you will join him if you continue behaving recklessly.”
Luca’s gaze became dangerous.
Elena saw the precise moment Isabella realized she had gone too far.
But before Luca could answer, a sharp sound came from the hallway.
A gunshot.
Then another.
The bodyguards shouted.
Elena instinctively moved in front of Luca.
The door burst open, and Matteo entered with his weapon drawn.
“Boss, we have a breach.”
Luca pulled Elena behind him despite barely being able to sit upright.
“How many?”
“Two men in hospital uniforms. One is down. The other disappeared near the service elevators.”
Isabella’s face turned pale.
Luca did not appear surprised.
“They came to finish it.”
Matteo nodded. “We need to move you.”
“The underground exit?”
“Possibly compromised.”
Luca looked at Elena.
“You’re coming with me.”
She stared at him. “I can’t leave the hospital.”
“They saw you in this room.”
“That doesn’t mean—”
“Celeste saw me holding your hand. My mother confronted you in a hallway filled with cameras. If the men who attacked me learn that I care whether you live, they will use you.”
“You barely know me.”
“That won’t matter to them.”
Matteo approached. “Boss, we have ninety seconds.”
Luca reached beneath his pillow and removed a small black pistol.
Elena had no idea how it had gotten there.
He checked it with calm efficiency and looked directly into her eyes.
“You saved my life,” he said. “Now every enemy I have will assume you belong to me.”
“I don’t belong to anyone.”
His expression softened.
“Good. Neither do I.”
Another gunshot echoed from the corridor.
The lights suddenly went dark.
Emergency alarms began screaming.
In the red glow from the backup system, Luca reached for Elena’s hand once more.
This time, she took his willingly.
Matteo opened a concealed door behind the wooden wall panel. Beyond it waited a narrow passage and two armed guards.
Luca stepped inside but stopped when Elena hesitated.
Behind her was the life she understood.
Her job.
Her apartment.
Her ordinary routines.
Ahead stood a wounded mafia king with blood on his bandages and an army waiting for his command.
“Once I walk through that door,” she whispered, “nothing will be normal again.”
Luca looked at her as gunfire erupted somewhere below them.
“Normal almost got you killed.”
Then he drew her close, shielding her body with his own despite his wounds.
“Stay with me, Elena, and no one in this city will ever touch you without losing the hand they used.”
She should have been terrified.
Instead, for one dangerous heartbeat, she felt safer beside him than she had ever felt anywhere else.
